It’s your money, so we help you design your budget spend and choose your support workers to suit exactly what you need, when you need it most.
Our Case Managers will work together with you to design a care package of coordinated care and services best suited to help you with your daily living. This could include a mix of the services included below, depending on your needs and your Home Care Package level.

Depending on the level of Home Care Package you receive, you can get assistance with a range of different health care services and home health aides.

What a Home Care Package (HCP) DOES NOT cover

There are certain services that Home Care Packages do not cover, these include:
General Expenses
Items that would normally be purchased out of general income
Good and Beverage
Buying food, except as part of enteral feeding requirement
Accommodation
Payment of permanent accommodation including mortgage or rent
Renovations
Home modifications or assets that are not related to your core needs will not be funded
Entertainment
Costs of entertainment activities, such as club memberships, sporting events or gambling
Holidays
travel and accommodations for holidays and travel outside your area of living
How to get started with an application for a Home Care Package.
Registration
Register on My Aged Care. You will be referred to an Aged Care Assessment Services (ACAS) team to evaluate your needs for funded support at home.
Eligibility
If eligible, you will be informed by letter on your approved level of funding. You will go into a national queue until a suitable package is available.
Research
While waiting for your package, research and find an approved service provider that best suits your needs and individual circumstances.
Agreement
Once your package becomes available, you have up to 56 days to enter into a service agreement with your chosen service provider.
Service Begins
If you have chosen Trilogy Care, one of our Care Advisers will meet with you to develop a care plan that helps you get the most value from your package.
